INTENT TO ENROLL & REGISTRATION

VISION PREPARATORY CHARTER SCHOOL 2022-2023 REGISTRATION INSTRUCTIONS

Welcome, Prospective Parents!

OPEN ENROLLMENT FOR 2022-2023

Vision Preparatory Charter School is now accepting applications for the 2022-2023 school year. The school will be continuing in its sixth year of operation and offers Kindergarten through 5th grade. The open enrollment period is from January 15 through March 1.
Interested families can click here ENROLL NOW to complete an Intent to Apply Form. If more applications are submitted than open seats available, a lottery will be held.
  • Step 1: Fill out the Intent to Enroll form for your scholar for the 2022-2023 school year, click here to apply online. This will initiate contact with Madrilyn Parham, our Parent and Admissions Coordinator; she will reach out to you to answer questions and assist you as you move forward in the process of joining our Vision Prep family.

Parents who have not completed an Intent to Enroll application and mandatory parent meeting will not be allowed to register. Contact Madrilyn Parham, Parent Coordinator for Admissions, at 901-775-1018 to schedule a parent meeting.

Parents must bring the following information in addition to the required registration paperwork:

  • A copy of student’s social security card

  • A certified copy of student’s birth certificate (Kindergarten students must be 5 years old on or before August 15, 2015)

  • TN Dept of Health Immunization Certificate

  • 2 documents showing Proof of Residence in Shelby County (parents who live with another person must provide a notarized Shared Residency Affidavit, which is provided in the registration packet):

    • TN driver’s license

    • Current MLGW bill

    • Copy of lease or mortgage statement

    • Copy of public assistance/government benefits

    • Copy of real estate property tax receipt

  • Rising 1st grade and 2nd grade students must have a copy of final report card showing promotion to their next grade level.

  • Kindergarten students must have proof of a physical exam within 12 months prior to enrollment (no earlier than 8-05-19).

  • Students currently receiving special education services must provide a recent copy of an IEP or 504 plan.
